Fieldwork Level I
  I will complete four Level I experiences.  Below are the facilities I visited:

November 2006
-Clarian Health Services, Methodist Hospital
-Indianapolis, IN
-Orthopedic and Neurologic Adult Inpatient
-CI:  Jenny Dickey, OTR
-While at this site, I was given the opportunity to interact with  the patients, as well as administer the Canadian Occupational Performance Measure with a patient.
 
March 2007
-Judevine Center for Autism
-St. Louis, MO
-Pediatrics
-CI:  Julie Grana, OTR
-While at this site, I had the chance to learn about children with severe behavioral and sensory issues.  I was given the opportunity to plan and implement two therapy sessions on my own.
 
October 2007
-Traci Tyler & Associates
-St. Louis, MO/Illinois
-Early Intervention/Home Health
-CI:  Dr. Erin Brown, OTR
-While at this site, I had the chance to observe early intervention with children ages 0-3.  I saw a wide variety of issues including, autism, feeding, sensory, etc.
 
March 2008
-Riverview Hospital Sport Fitness and Rehabilitation Services
-Indianapolis, IN
-Outpatient Services for Adults and Children
-CI:  Kate Hansen and Nikki Maxwell
-While at this site, I had the chance to observe a variety of upper extremity injuries.

-Comprehensive Inpatient Rehabilitation Unit

Acute Care Fieldwork
For my Acute Care Experience, I attened Barnes Jewish Hospital in St. Louis on the following dates:

  • October 3, 2006  - Observation
  • October 24, 2006 - I administered the Canadian Occupational Performance measure on a patient chosen by my Acute Care Instructor.
  • January 30, 2007 - I observed a stroke patient while a therapist performed the Barthel.
  • February 20, 2007 - I scored a client using the Barthel.
